All forms must be submitted to Mona Abdulla in the Student Activities Office on the second floor of the Seahawk Center.

 

Banner Order Form

Do you need to get a message out?  Order a banner from Student Activities to publicize your event/activity/program.

Facilities Request Form

This form allows you to reserve the Seahawk Center or the Quad area along with equipment.

Complaint Form

If you have a grievance please fill out a complaint form.  Issues of grievance can include, but are not limited to: harassment and discrimination.

Request to Distribute Materials/Free Speech Form

The area surrounding the flagpole is our free speech area.  Anyone wishing to exercise their right to free speech and distribute informational materials may do so after submitting this form to Student Activities.

Academic Honor Certificate Request Form

The Seahawk Center hosts the Academic Honors Awards Ceremony every spring.  If you do not attend the ceremony and wish to request a copy of your Academic Honor Certificate you must submit a request form to Student Activities.

Application for Elected Office

Elections for ASO President, Executive Vice President, and Vice President of Finance are held every spring.  If you wish any one of these office return this application to Student Activities.  Election deadlines, election packets, and any other pertinent information will be made available through the ASO website.

Application to Distribute Materials and/or Free Speech

Organizations/individuals may distribute materials and/or exercise their right to free speech in the free speech area of the Seahawk Quad.  Such organization/individuals include the following: non-profit, political, religious, and educational.  This form must be submitted to Student Activities and all regulations outline on the form must be adhered to.

Los Angeles Harbor College Vendor Registration

Vendors wishing to conduct business at Los Angeles Harbor College may reserve space in the Seahawk Quad.  Vendors must register with Student Activities.  Small businesses pay $40.00 per day.  Large businesses pay $100.00 per day.
