General Petition - This is our most commonly requested form. This is commonly used for Academic Renewal, to have credit recognized from out of state schools, and a variety of other reasons.
Petition for Grade Review - If you feel that there is a problem with the grade you received, you may complete this form and submit it to Admissions and Records.
Grade Report / Work In Progress - Use this form to have your professors tell you your current grade in the class. This is commonly used for students facing Probation or Dismissal.
Student Complaint Form - If you have experienced a problem at Harbor College, you may fill out this complaint form. Completed forms may be turned in to the President's Office in the Student Services building.
Course Repetition for GPA Adjustment - If you have have repeated a class and would like your GPA adjusted to reflect the new grade, you may turn this form in to the Admissions and Records Office.
Proxy Form - Use this form if you are having someone else submit forms or transcript requests on your behalf.
