Application Procedure

Remember the following before submitting the application:

  • Only fully completed applications can be considered
  • All letters of recommendation must be included in the application.
  • A complete application consists of these items:
    • Application form with all questions that apply answered completely.
    • A copy of parent/guardian’s latest income tax Form 1040 (if filed), OR
    • Income verification form in this application (for those who did not file a 1040).  The verification form must be Notarized.
    • ____ Official high school transcript.
    • ____ Available test scores from your school.
    • ____ Two (2) recommendations (forms attached)
    • ____ One (1) counselor
    • ____ One (1) teacher (prefer hard subject courses)

    NOTE:  Make sure that both student and parent/guardian sign as directed.

      As there is only a limited number of students who are able to be served by the LAHC Upward Bound Program, do not miss this opportunity by failing to complete the application.
